INSTAGRAM_ACCESS_TOKEN is set, but Instagram did not return usable media. Restart npm run dev after editing env, confirm INSTAGRAM_USER_ID matches your professional account, and open /api/instagram/feed?debug=1 locally to see Meta error hints. Tokens from Graph API Explorer are often short lived (~1 hour). You can still add manual posts in data/instagram-posts.ts.
